Disc Herniation Treatment in Fountain Valley, CA

A disc herniation means disc material has pushed through a weakened annulus and can irritate a nearby nerve or the tissues around it. Irritation can be chemical, mechanical, or both. An MRI sentence is not the whole story. Care often uses directional movement, and many people are treated without surgery. For patients coming from Fountain Valley, the office is about 20 miles away, 30–45 minutes, by the 405 south to the 73 toward Mission Viejo.

Annular injury and two ways a disc irritates

The annulus is the layered wall of the disc. When it fissures, nucleus material can bulge, protrude, or extrude toward the canal or a nerve root. That mechanical contact can hurt. Chemical irritation from inflamed disc material can sensitize the root even when the fragment is not crushing it.

You do not need to memorize the grades. You do need to know that pain, numbness, and a movement preference are clinical facts, while the shape on a scan is another fact. Lower back pain can exist with a small annular fissure, and a large herniation can be quieter than its picture.

An MRI finding is a clue, not a biography

Disc changes are common in adults who are not seeking care. If your report says herniation, the next question is whether today's symptoms and exam match that level. A report cannot feel your reflex, watch repeated bending, or tell whether pain moves out of the leg.

Dr. Tara Kohen, D.C. and Dr. Sani Kohen, D.C. read imaging when you bring it, then put it beside the movement exam. If the two disagree, the plan follows the person in the room. Chasing a phrase on the film is how people get treated for a finding that was never their pain.

Centralization during McKenzie testing

The McKenzie Method uses repeated end-range movements to see whether symptoms centralize, peripheralize, or stay put. Centralization, pain leaving the calf or foot and gathering toward the spine, is a reason to keep that direction. Peripheralization is a reason to stop it.

A directional preference is not a personality type. It can change as irritability drops. Retesting is the method. If no direction helps, the disc story may still be true, and the plan leans on unloading, neurodynamics, and a slower introduction of movement instead of forcing extension because a textbook likes it.

Most discs are managed without an operation

Most people with a symptomatic herniation are treated without surgery. That is a general clinical reality, not a percentage promised to you, and not a claim that every disc quiets on a schedule. Progressive weakness, cauda equina features, or pain that will not yield are reasons to involve a surgeon.

New bowel or bladder changes, saddle numbness, rapidly worsening weakness, fever with spine pain, or significant trauma belong in emergency care, not a routine visit. For the rest, sciatica care at Functional Restoration Institute stays inside Chiropractic Sports Medicine and Neuromuscular Rehabilitation, with adjustments only when the exam supports them.

What to expect when you are seen for Disc Herniation

The first hour is mostly repeated movement and a neurologic check of the leg. You will be told, in plain words, whether a direction pulled symptoms toward the back or pushed them farther away. Home work is that direction, done as prescribed, not a long list of stretches.

Later care may add mobilization or soft tissue work around a segment that will not move, plus control drills once the leg is calmer. The office is out of network and can issue a superbill for HSA or FSA submission. No visit number is offered as a finish line.

Mile Square, a flat grid, and long freeway days

Fountain Valley is flat, which sounds simple and is not. Mile Square Park gives people a long, level loop for walking and cycling. The rest of the city is a grid of Slater, Brookhurst, and Euclid: short drives, a lot of sitting, and jobs that may be in the city or up the 405. A flat park loop is a clean test of walking tolerance. Pain that waits until the car, not the loop, points us at sitting and at the spine rather than at a hill that does not exist here.

Low back pain and sciatica are evaluated with that distinction. Neck pain often belongs to the 405. Knee pain in cyclists around Mile Square gets a bike-and-hip history, not a generic brace.

405 to the 73

Take the 405 south to the 73 toward Mission Viejo, then to La Alameda. About 20 miles and 30 to 45 minutes. The address is 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. Park at the building. Hours run Monday through Thursday, 9 to 6. Leaving Fountain Valley after 4 and expecting a calm neck exam is optimistic. Book earlier.

Techniques and medical problems we will and will not own

Neurodynamics and McKenzie Method care are central for nerve and disc patterns. Gait analysis earns its place when the Mile Square loop and the sidewalk tell different stories. Cupping is a decompression tool, not a detox.

We also see metabolic syndrome and diabetes questions beside the physician who manages medication. We do not change those drugs. The city page is the Fountain Valley chiropractor page.

Payment

Out of network. Superbill. HSA and FSA. No referral. Bring a medication list if you want labs or if you are here for a systemic symptom. Imaging is helpful and not required for every back pain visit.

Questions patients ask

What is the difference between chemical and mechanical disc irritation?

Mechanical irritation means disc material is physically contacting or compressing a sensitive structure, often a nerve root. Chemical irritation means inflammatory material from the injured disc is sensitizing that nerve even without a hard pinch. Both can exist together. The exam cannot see the chemistry, but it can track whether movement changes the pain map.

Why can my MRI look serious when my symptoms are mild?

Disc herniations and bulges show up in people who have little or no pain, so the picture is not a diary of how you feel. A large fragment can sit away from your symptomatic root, and a small fissure can be extremely sensitive. Your strength, reflex, and movement response tell us whether the sentence on the report is the problem we should treat.

What does centralization mean while you test a herniated disc?

During repeated movements, centralization means the pain or tingling retreats from the foot, calf, or thigh toward the center of the low back. That response suggests the direction is useful right now. It is retested because it can fade or flip. It is not proof the herniation has vanished, and it is not a warranty.

Does a herniated disc mean I should plan on surgery?

No. Most people are treated without an operation, using directional exercise and a careful return to load. Surgery becomes a serious conversation when weakness is progressing, when emergency neurologic signs appear, or when a well-run course of conservative care fails and a surgeon agrees the anatomy matches. Functional Restoration Institute does not perform surgery.
