Thyroid, glucose, sleep, hormones, and muscle
A slow thyroid, sharp glucose swings, short sleep, and a hormone pattern that disrupts cycles can each make fat loss feel stalled. Muscle changes the picture too. If you have lost strength, the goal is not only a lower scale number. Drs. Sani Kohen, D.C. and Tara Kohen, D.C. ask which of those is actually true for you.
The thyroid dysfunction page covers pace and a full panel. Metabolic syndrome covers the cluster of waist, blood pressure, glucose, triglycerides, and HDL that your physician may already use. Weight sits among those facts. It is not a separate moral category.
When pain is the reason training stopped
You cannot follow a strength plan if a knee, a hip, or the low back stops you. Corrective exercise therapy rebuilds the movements you need. Chiropractic Sports Medicine looks at the joint. Neuromuscular Rehabilitation looks at control. Skipping that and talking only about food leaves the barrier in place.
Pain also steals sleep, and short sleep makes appetite harder to steer the next day. That loop is practical. It is not fixed by a stricter set of rules while the joint still wakes you. Say which movement you avoid.
Labs that explain the stall, without a promise
Advanced bloodwork may include glucose markers, lipids, and a thyroid panel when the history points there. DUTCH hormone testing is for a hormone question, such as cycle change or a cortisol pattern, not a requirement for everyone who wants to lose weight.
FRI does not promise pounds, inches, or a date. It does not make GLP-1 claims, and it does not prescribe those medications. If a physician has prescribed one, you do not stop it because of a visit here. Nutrition is built from your labs and your schedule.
Food and strength on one plan
Protein, meal rhythm, and enough food to train are discussed without a branded diet. Under-eating while adding hard sessions is a common way people feel worse and call it a plateau. Nutrition and supplementation stays specific. There is no dose list on this page.
Functional medicine at FRI does not replace a primary physician, endocrinologist, gastroenterologist, or mental health clinician. Disordered eating needs that clinical care, not a tighter meal plan from this office. The visit will ask, and it will not push past a real eating disorder.
What to expect when you are seen for Weight Loss
A complimentary 20-minute consultation can decide whether the work is labs, joints, or both. FRI is out of network. A superbill is available, and HSA or FSA funds are accepted. Hours are Monday–Thursday, 9–6.
You will not be given a goal weight or a promised loss. Expect questions about sleep, cycles, pain, and prior labs. Training starts where your joints allow, not where a program template starts.
Money, time, and what the first hour is for
FRI is out of network. Huntington Beach patients commonly use HSA or FSA cards and submit a superbill. Ask your plan what it does with out-of-network chiropractic and lab work before you assume a number. Lab tests, when they are ordered, are a separate cost from the visit. We will tell you what we want to order and why before you are committed to it.
The evaluation is the product. You should leave knowing what we think is going on, what we are not worried about, and what you are supposed to do before the next visit. If the honest answer is that another clinician is the better next step, that is the plan. Drs. Sani and Tara Kohen both see patients here. Their training is described on the about page, and the building is shown on the office page.
Surf, sand, and the inland drive from Huntington Beach
Huntington Beach lives on the water in a different way than the harbor cities. Downtown and the pier, Bolsa Chica, Seacliff, and Huntington Harbour each produce a different week. Surfing, beach volleyball, and long walks on the sand load the feet, knees, and low back. A desk job inland, or a shift that ends with PCH traffic, loads the neck. We ask about that sequence because a shoulder that hurts after a dawn patrol is not examined the same way as a shoulder that hurts after a day at a laptop. The sand itself matters: barefoot time on an unstable surface changes what we look at in a gait exam.
The drive is the other half of the week. Getting from Main Street or from the harbour to south county is a real trip, about 22 miles, often 30 to 45 minutes on the 405 south to the 73. People who make it have usually decided they want an exam that lasts longer than a lunch break. If you are closer to a high-volume office on Beach Boulevard and you still drive inland, say so. It helps us understand what you already tried.
Arriving at La Alameda from the 405
From Huntington Beach, the usual path is the 405 south, then the 73 toll road toward Mission Viejo, exiting toward La Alameda. The suite is 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. Parking is at the building. The clinic is open Monday through Thursday, 9 to 6. A mid-morning visit is the least painful return to the coast. An end-of-day visit means sitting in 405 traffic with whatever we just asked your back to do, so we will tell you if a same-day training session is a bad idea.
Call (949) 312-1754 or book the complimentary 20-minute consultation online if you want to test the fit before a full evaluation. Bring prior imaging if you have it, but do not delay the visit waiting for an MRI you have not been told you need. Many spine and joint problems can be examined without new pictures. If the story sounds like something that needs a physician or an emergency department first, we will say that plainly.
Questions patients ask
How many pounds will I lose?
FRI does not promise a number of pounds, a timeline, or a clothing size. Weight is discussed beside thyroid, glucose, sleep, hormones, and muscle. Some people see the scale change. Some see strength and labs change first. Neither is promised. If a physician is managing a medical weight treatment, that plan stays with them.
Do you prescribe GLP-1 medications?
No. This page is not about GLP-1 drugs, and FRI does not make claims about them. If your physician prescribed one, do not stop it based on a visit here. The work in this office is nutrition, labs, sleep, and training you can actually do. Questions about those medications belong with the prescriber who monitors them.
What if pain keeps me out of the gym?
Then pain is part of the weight conversation. Corrective exercise therapy, Chiropractic Sports Medicine, and Neuromuscular Rehabilitation look at the joint and the pattern that fails. A food plan that assumes you are lifting four days a week is the wrong plan if you cannot stand up from a chair without a sharp hip. The barriers get named.
Will a thyroid test be ordered automatically?
Only if the history suggests it. Cold intolerance, cycle changes, hair and bowel shifts, or a prior borderline result are reasons. A person with none of those may not need a thyroid panel to talk about meal rhythm and a sore knee. DUTCH testing is similarly reserved for a hormone question, not added because weight was mentioned.
