Blood for recent contact, urine for excretion
Blood levels are most informative for recent or ongoing exposure. They do not catalog a lifetime of contact. A urine collection shows what is being excreted over the collection window, which answers a different question. Hair and provoked tests exist in the wider marketplace. This office does not build a plan on a provoked urine after a chelating drug. That is a chelation procedure, and it is not offered here.
Heavy metal testing starts with which metal and why. Lead, mercury, arsenic, and cadmium are examples that come up because of a real exposure, not because a panel is long. Dr. Sani Kohen would rather order the metal the history suggests than a souvenir printout. The lab is an additional cost, explained before the draw or the jug is ordered.
Work, water, fish, and dental history
The history is the test before the test. Welding, battery work, old paint, well water, large predatory fish several times a week, and certain hobbies change which sample makes sense. Dental work is asked about as history, not as an order to remove fillings. Removal is a dentist's decision. FRI does not schedule chelation around amalgam.
Heavy metal toxicity symptoms people describe include fatigue, brain fog, and numbness. Those symptoms have many causes. The exposure story is what makes a metal lab rational. Chronic inflammation without any exposure history is a weak reason to draw a metal panel. The question has to be able to survive being said out loud.
A number without a chelation plan
If a result is high, the first actions are to stop ongoing exposure and to involve the right physician. Chelation drugs are a medical procedure with risks. They are not a Functional Medicine add-on in this clinic, and no protocol for them is written here. You will not leave with a schedule of chelating agents.
A mildly abnormal number in a person with no exposure and no matching illness is not chased for its own sake. Labs vary. Repeat testing, a different specimen, or a referral may be the honest next step. Dr. Tara Kohen will say when a result is concerning and when it is a quirk. Treating every quirk is how people end up on drugs they did not need.
What the visit actually decides
The review often runs 30 to 60 minutes depending on the plan. You look at the metal that was ordered, the units, and the exposure that prompted it. Next steps might be a water test you arrange yourself, a change in fish intake, occupational follow-up, or a physician referral. They are not a bottle.
A free 20-minute consultation can decide whether any metal lab is warranted. The office is out of network, accepts HSA and FSA cards, and provides a superbill for the visit. The laboratory fee is separate. If your employer or a public health agency should be the one testing, you will hear that too. Not every exposure question belongs in a private functional panel.
What a Heavy Metal Testing visit is like
Expect detailed questions about work, water, diet, and hobbies before any tube is filled. If the history does not support a lab, the lab is not ordered to be thorough. Collection might be a blood draw, a timed urine, or both, and the instructions for urine are easy to ruin with a missed sample. Read them the day before.
The review will not include a chelation calendar. If a result needs urgent medical care, that is the recommendation. If it needs a practical change in exposure, that change is named. You should be able to repeat, in your own words, what was tested and why the other metals were left alone.
How care is organized once you are in the office
The first full visit is an evaluation. For pain and injury that means a history, movement testing, and the hands-on exam that fits the complaint, which may include joint motion, nerve tension, and orthopedic tests. You leave with an explanation of what we think is driving the problem and a short plan, often including something you do at home. Adjustments, soft tissue work, and exercise are tools inside that plan. They are not a package you are expected to buy before anyone has examined you.
If the main issue is hormones, digestion, energy, or an autoimmune diagnosis you already carry, the visit leans on history and on which labs are actually worth running. DUTCH, GI-MAP, thyroid panels, and the rest of the functional lab menu are chosen from the story, not ordered as a bundle. Medication changes stay with the physician who prescribed them. FRI is out of network with most plans. Many Newport patients pay with HSA or FSA funds, and we provide a superbill for whatever reimbursement your plan allows.
Where Newport patients usually go next on this site
People driving up from the peninsula and Corona del Mar often start with neck pain or shoulder pain after paddling and desk time, or with lower back pain after long sits in the car. Harbor athletes looking for a tissue exam rather than a generic rub sometimes book Active Release Technique or a gait and postural look when the foot and hip are part of the story.
Others are not here for a joint at all. Perimenopause and gut health questions are common alongside training, and the matching lab pages are DUTCH hormone testing and GI-MAP stool testing. If you want the wider map of the city, the Newport Beach chiropractor page ties the three pillars together. The doctors' backgrounds are on the about page.
What a Newport Beach week actually asks of the body
Newport Beach is not one neighborhood. Corona del Mar climbs away from the water, Balboa Peninsula is flat and windy, Newport Coast sits on hills above the toll road, and the streets around Eastbluff, Big Canyon, and Fashion Island mix short drives with long desk blocks. People paddle in the harbor, ride the Back Bay loop, play tennis, or swim before a day of meetings. That mix shows up in the history we take: which shoulder loads a paddle, which hip complains on the hills, which neck tightens after a commute that ends at a screen. None of those details is a diagnosis. They tell us which movements to test.
Food and schedule questions belong in the same visit when symptoms are not only mechanical. A late dinner after a harbor event, an early paddle, and a short night are ordinary Newport weeks. If you are also dealing with cycle changes, reflux, or a fatigue that does not match your training, the Functional Medicine side of the clinic can look at labs chosen from that story. The manual care and the lab conversation happen in the same practice, so you are not retelling the history in two buildings.
Questions patients ask
Will you chelate me if the level is high?
No. FRI does not provide a chelation protocol. High results lead to stopping exposure and involving the appropriate physician. Chelating drugs are not part of this visit.
Does blood show my lifetime metal burden?
Blood is better for recent contact. Urine shows excretion over the collection. Neither is a complete biography of every exposure you have ever had.
Should my fillings come out because of a mercury test?
That is a dentist's decision, not a conclusion from this visit. Dental history is recorded. Removal is not scheduled from a functional lab.
Is the lab included in the visit?
No. Testing is an additional cost, discussed before it is ordered. The review visit is separate and often runs 30 to 60 minutes depending on the plan.
