Median nerve irritation at the wrist
The median nerve crosses the wrist under a fibrous roof. Swelling, sustained wrist flexion, or a tunnel that is simply tight can irritate it. The classic map is the thumb, index, middle finger, and part of the ring finger, not the little finger. Little-finger numbness suggests a different nerve and a different exam.
Grip and pinch that fade with use, and tingling that wakes you and eases when you shake the hand, fit this pattern. A sore forearm muscle can coexist and is not the nerve. Active Release Technique may address that muscle. It does not get to skip the neurologic questions.
Night symptoms, grip, and the thenar muscles
Night symptoms matter because the wrist often rests flexed, and the nerve's blood supply is easier to embarrass in that position. A neutral wrist position at night is a reasonable early experiment. It is not a cure, and it does not tell you how severe the compression is.
Look at the mound of muscle at the base of the thumb. Flattening there, or weakness that lets the thumb collapse during pinch, is thenar involvement. That finding should also be seen by a physician. Mild tingling without weakness is a different urgency than a wasting muscle.
When the neck imitates the wrist
Neck pain and a cervical nerve root can send tingling into the hand, sometimes in a similar fingers-and-grip story. If neck motion changes the hand symptoms, the wrist is not assumed guilty. Both regions can be irritable at once, a situation clinicians call double crush, and each still has to earn its place.
The cervical screen includes movement, a compression test if it is safe, and a check of reflexes and strength above the wrist. Neurodynamics for the median nerve then asks whether gliding the nerve changes the familiar tingling without requiring you to hang on a painful stretch.
When a physician should see the hand as well
Severe weakness, thenar wasting, constant numbness, or symptoms that are rapidly worsening should be seen by a physician, who may discuss nerve studies or a surgical opinion. This office does not perform carpal tunnel release. Waiting on soft tissue care alone in that situation is not a favor.
For milder, intermittent symptoms, care may include nerve glides, forearm soft tissue work, and a change in sustained wrist posture. Chiropractic Sports Medicine and Neuromuscular Rehabilitation share that plan at Functional Restoration Institute. Spreading numbness or new weakness pauses it for medical review.
What to expect when you are seen for Carpal Tunnel Syndrome
The hand map, night pattern, grip, and neck are checked in one visit so the wrist is not blamed by habit. You may be shown a gentle median glide and a neutral night position. You will be told plainly if the thumb muscle needs a physician.
Follow-ups ask whether nights are quieter and whether grip is more dependable, not whether the hand feels interesting after a technique. If symptoms are climbing despite those changes, the plan refers outward rather than adding more sessions of the same work.
Alicia, Pacific Park, and the 73
From the Town Center, Alicia Parkway toward Mission Viejo is direct. From the south and west edges, Pacific Park or the 73 north to a Mission Viejo exit also works. About 7 miles and 12 to 20 minutes when the parkways move. The suite is 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. On-site parking. Monday through Thursday, 9 to 6.
If your day already includes the 73 toll road to Irvine, stacking a clinic visit on the south end of that drive can be efficient. Do not book so tight that you walk in from a stressful drive and expect a meaningful neck exam. Call (949) 312-1754. The complimentary consultation is 20 minutes if you want to start smaller.
Care that fits a canyon-and-commute city
Neck pain, lower back pain, and headaches track with the Irvine commute. Hip and SI pain tracks with hills and carrying kids up them. Joint mobilization and corrective exercise are everyday tools. McKenzie care is the spine option when symptoms have a direction that centralizes them.
Hormone and gut visits are part of the same practice. Perimenopause and SIBO have their own pages so those questions are not buried under sports advice. The Aliso Viejo chiropractor page is the hub.
Straightforward office policies
No referral required. Out of network. Superbill. HSA and FSA. We do not invent a copay. Labs, if ordered, cost extra and are explained first. Medication stays with your prescriber.
You will see Drs. Sani and Tara Kohen's practice, described on the about page. Photos of the gym and rooms are on the office page. If a problem is outside chiropractic and functional medicine, we will say so rather than stretch the visit to cover it.
Town Center days and Aliso Canyon edges
Aliso Viejo is compact, master planned, and hillier than it looks from the 73. The Town Center is errands, sitting, and short walks. The neighborhoods toward Aliso and Wood Canyons add dirt descents and stairs. A lot of residents commute north to Irvine business parks and then come home to a hill. Necks take the office. Knees take the canyon. We want both halves of that day, because treating only the part that hurts ignores the part that loaded it.
The canyon trails are a gift and a test. People start plantar fascia or knee complaints after they add mileage on dirt while their week is otherwise a chair. Gait analysis and a loading plan are more useful than a month of rest followed by the same trail.
Questions patients ask
Why do carpal tunnel symptoms often show up at night?
Wrists commonly rest in flexion during sleep, which raises pressure in the tunnel and bothers the median nerve. Tingling in the thumb, index, and middle finger that wakes you, then eases when you shake the hand, fits that pattern. A neutral wrist position is a fair trial. It does not measure severity, and daytime grip weakness still needs an exam of its own.
Can a neck problem copy carpal tunnel syndrome?
Yes. A lower cervical root can tingle into the hand and disturb grip, and neck motion may be what actually reproduces it. The wrist exam still happens, because both sites can contribute. If the neck copies the symptoms and the wrist tests are quiet, treating only the carpal tunnel misses the driver. If both are positive, the plan says so instead of picking a favorite.
What is neurodynamic work supposed to do for the median nerve?
It moves the nerve relative to the tissues around it with a gentle glide, looking for a change in the familiar tingling rather than a strong stretch. The dose is small. More tension that leaves the fingers electric an hour later is too much. Glides are paired with posture changes and soft tissue work. They are not a reason to ignore thumb weakness.
When should a physician also examine my hand?
If the thenar muscles are flattening, pinch is clearly weak, numbness is constant, or symptoms are worsening quickly, see a physician as well. Nerve studies and a surgical opinion live in that lane, and this clinic does not do carpal tunnel surgery. Mild intermittent night tingling can reasonably start with a movement exam. Wasting and progressive weakness should not wait on it.
