Perimenopause Treatment in Rancho Santa Margarita, CA

Perimenopause is the transition while cycles are still happening, often unevenly. Sleep can break, mood can swing, and joints can ache even though you have not gone a full year without a period. This page treats that stage as its own pattern, with hormone mapping and a clear line between this visit and menopause. For patients coming from Rancho Santa Margarita, the office is about 7 miles away, 12–20 minutes, by Santa Margarita Parkway or Antonio Parkway to Mission Viejo.

A transition with uneven cycles

Cycles may shorten, then stretch, then arrive twice in one month. Flow can be heavier than it used to be, or spotting can show up midmonth. You are still bleeding, which is the practical line between this stage and menopause. That line matters because the hormone pattern is usually swinging, not simply gone.

Drs. Sani Kohen, D.C. and Tara Kohen, D.C. ask for dates, not a single story about getting older. Functional Medicine looks at how those swings line up with sleep, temperature, and energy. The visit does not hand you a bioidentical dose.

Sleep, mood, and joint aches while cycles continue

You might fall asleep and wake hot at 2 a.m., then feel flat or irritable the next afternoon. Word-finding can slip. Shoulders, hands, and hips can ache in a way that feels new. Those complaints are common in this transition, and they still deserve a check for thyroid, pain, and mood care you already have.

A page on sleep and insomnia covers caffeine, pain, and night waking in more detail. If worry or low mood is the main problem, keep your mental health clinician. FRI can look at sleep and cycle hormones as contributors. It does not provide therapy.

This stage is not menopause

Menopause is marked, in usual clinical practice, by twelve months without a cycle. Perimenopause is the uneven stretch before that. Treating them as the same page leads to the wrong expectations about bleeding, bone, and what a lab can explain. The menopause page is separate for that reason.

DUTCH hormone testing can show estrogen and progesterone metabolites and the cortisol curve while your cycles are still arriving. Timing the collection to the cycle matters. A random blood draw on an unspecified day often answers a narrower question than the one you asked.

What the visit will and will not decide

FRI can talk through symptoms, a hormone pattern, nutrition, and strength work if joints are limiting you. Chiropractic Sports Medicine and Neuromuscular Rehabilitation are available when pain is part of the month. Hormone prescriptions, including any bioidentical therapy, stay with the clinician who would prescribe them.

Functional medicine at FRI does not replace a primary physician, endocrinologist, gastroenterologist, or mental health clinician. Unexpected heavy bleeding, chest pain, or a sudden severe headache needs medical care, not a urine kit. Nutrition and supplementation is individualized, without a brand protocol.

What to expect when you are seen for Perimenopause

Start with a complimentary 20-minute consultation if you want to see whether the timing is right. FRI is out of network and can provide a superbill. HSA and FSA funds are accepted. Visits are Monday–Thursday, 9–6.

Bring a recent cycle log and a list of prescriptions. If a DUTCH collection is used, you will be told which days matter. Expect a plain-language review, not a promise that flushes or sleep will stop on a date.

Dove Canyon, O'Neill trails, and a planned-town schedule

Rancho Santa Margarita spreads into Dove Canyon, Robinson Ranch, Tijeras Creek, and the central town. O'Neill Regional Park and the surrounding trails add climbing that a map of the shopping district does not show. Golf and youth sports fill weekends. Weekdays are often a drive out Antonio or Santa Margarita toward a desk somewhere else. Knees and hips meet the trails. Necks and low backs meet the car. We ask which village you live in because the grade and the drive are not the same in Dove Canyon as they are next to the lake in town.

Lower back pain after the drive out of the hills, and knee pain after O'Neill, are typical reasons to come to La Alameda. Scoliosis questions come from parents who want movement help without a promise that exercise will straighten a structural curve. We will not make that promise.

Santa Margarita Parkway to La Alameda

Santa Margarita Parkway into Mission Viejo, or Antonio Parkway to local streets, covers the trip. About 7 miles and 12 to 20 minutes when the parkway is moving. School traffic changes that. The suite is 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. Park at the building. Monday through Thursday, 9 to 6.

You are close enough to do a real course of care without giving up half a day. Use that. Spacing visits so you can practice the home plan is more valuable than stacking them. Call (949) 312-1754 or begin with the free 20-minute consultation.

Hands-on care and the lab side

Functional movement screening and gait analysis fit a town that hikes and then sits. ART is the soft-tissue option for overuse from sport, not a spa massage. Neurodynamics belongs when leg or arm pain behaves like a nerve.

For non-joint problems, sleep, hormonal imbalance, and gut health each have a page. The city starting point is the Rancho Santa Margarita chiropractor page.

Policies that do not change because you are close

Out of network. Superbill. HSA and FSA accepted. No referral required. We coordinate with physicians and we do not edit their prescriptions. Emergency spine and neurologic symptoms go to the ER.

Drs. Sani and Tara Kohen practice together at FRI. Read the about page and the office page if you want the people and the building before you drive Santa Margarita Parkway.

Extra context on Perimenopause

Track one full cycle, or two if they are short. Mark bleeding, spotting, night sweats, and the nights you were awake without heat. Add irritability, breast tenderness, and which joints complained. That single page keeps a lab from being interpreted in a vacuum. If your cycles are very far apart, say so before a collection is scheduled, because timing instructions change. Bring any prior hormone therapy, including creams, even if you stopped them. Hidden exposure makes a metabolite pattern hard to read. Dates matter more than a general sense of getting older.

Strength work and protein become more relevant when you notice clothes fitting differently and joints complaining on stairs. The conversation is about keeping muscle while sleep is unstable, not about a contest. If pain is the barrier, it is assessed directly rather than blamed on hormones alone. Functional Medicine, Chiropractic Sports Medicine, and Neuromuscular Rehabilitation can share a plan. None of them replaces the physician who would manage bleeding problems, hormone therapy, or a mood disorder. Joint pain still gets its own exam rather than a hormone explanation by default.

Questions patients ask

Am I in perimenopause if I still get periods?

Often that is exactly the situation. Perimenopause is the transition while cycles continue, sometimes in a messy pattern. Menopause is the point after twelve months without a cycle. FRI does not assign those labels in place of your physician. The visit uses your bleeding dates, symptoms, and, when useful, a DUTCH map to describe the pattern you are in.

Can hormone swings affect sleep and mood?

Yes. Falling and rising estrogen and progesterone can line up with night sweats, 2 a.m. waking, and a shorter temper. A cortisol curve can add another layer if you are also under heavy stress. FRI looks at those contributors. If mood symptoms need care, keep your mental health clinician. This office does not replace therapy or psychiatry.

Will you start me on hormone replacement?

No. FRI does not set bioidentical or other hormone doses. If hormone therapy is something you want to consider, that decision and the monitoring belong with your prescribing physician. The work here is history, optional DUTCH testing, nutrition, sleep, and movement when joints hurt. You leave with context, not a prescription pad.

Why do my joints hurt now?

Aching joints are a common report in this transition, and they are not automatically a hormone deficiency that a pill must fix. Old injuries, training errors, and thyroid pace can sit in the same month. Neuromuscular Rehabilitation can address how you load a sore shoulder or hip. New swelling or hot joints should be seen by your physician.
