Sciatica Treatment in Fountain Valley, CA

Sciatica, used carefully, means leg pain from irritation of a lumbar nerve root. The pain often follows a dermatome and may arrive with numbness, a reflex change, or weakness in a matching muscle. Plenty of buttock and leg pain is not sciatica. This page is about how that distinction is made and when imaging actually helps. For patients coming from Fountain Valley, the office is about 20 miles away, 30–45 minutes, by the 405 south to the 73 toward Mission Viejo.

Leg pain that follows a nerve root

A lumbar nerve root can refer pain along a strip of the thigh, calf, or foot. L4, L5, and S1 are the roots you will hear named most often, because they cover the common maps down the leg. Pain that stays in a vague patch of buttock, with a normal neurologic exam, is not automatically one of those roots.

Dermatomal pain is only one clue. Numbness in the same strip, a reduced reflex, or weakness in the muscles that root supplies makes the nerve-root story stronger. Dr. Sani Kohen, D.C. and Dr. Tara Kohen, D.C. write those findings down rather than treating every radiating ache as sciatica.

Why plenty of leg pain is not sciatica

Hip joint pain, sacroiliac referral, local hamstring injury, and peripheral nerve irritation can all travel into the thigh. A disc herniation can cause true radicular pain, and so can spinal stenosis, but the label still depends on the exam, not on the word you use when you book.

Neurodynamics testing asks how your symptoms change when the nerve is lengthened and then slackened. If the leg ache does not follow that mechanical change, and strength and reflexes are intact, the root is a weaker explanation. The visit then looks at joints and muscle instead of insisting on a nerve name.

When a scan helps and when it waits

Imaging is not the opening move for a first episode of leg pain that is already being sorted by history and movement. An MRI becomes more useful when weakness is progressing, when a red flag is present, or when a surgeon may need pictures because conservative care is not the right path.

A scan that shows a disc bulge does not, by itself, prove the bulge is the source of today's pain. Many adults carry disc changes without matching symptoms. The McKenzie Method response and the neurologic exam still have to agree with the picture before anyone treats the film.

Treating the root without guessing the picture

If a repeated movement centralizes the leg pain, that direction becomes the first exercise, often before any long discussion of surgery. Chiropractic adjustments or mobilization may be added when a stiff segment is blocking that direction. Nerve gliding is dosed gently so the root is moved, not picked on.

New bowel or bladder changes, saddle numbness, rapidly worsening weakness, fever with spine pain, or significant trauma belong in emergency care, not a routine visit. Functional Restoration Institute will not hold those symptoms for a later opening. Chiropractic Sports Medicine is the pillar that frames this exam.

What to expect when you are seen for Sciatica

You should leave the first visit knowing whether your leg pain behaved like a nerve root, and which finding supported that. You may be given a single repeated movement or a gentle nerve glide, with instructions to stop if the pain spreads farther down the limb.

Follow-up visits recheck strength, reflex, and the map of the pain. A complimentary 20-minute consultation can be used if you only want to ask whether this sorting process fits your situation. No one will promise that the leg will be quiet by a certain visit.

Techniques and medical problems we will and will not own

Neurodynamics and McKenzie Method care are central for nerve and disc patterns. Gait analysis earns its place when the Mile Square loop and the sidewalk tell different stories. Cupping is a decompression tool, not a detox.

We also see metabolic syndrome and diabetes questions beside the physician who manages medication. We do not change those drugs. The city page is the Fountain Valley chiropractor page.

Mile Square, a flat grid, and long freeway days

Fountain Valley is flat, which sounds simple and is not. Mile Square Park gives people a long, level loop for walking and cycling. The rest of the city is a grid of Slater, Brookhurst, and Euclid: short drives, a lot of sitting, and jobs that may be in the city or up the 405. A flat park loop is a clean test of walking tolerance. Pain that waits until the car, not the loop, points us at sitting and at the spine rather than at a hill that does not exist here.

Low back pain and sciatica are evaluated with that distinction. Neck pain often belongs to the 405. Knee pain in cyclists around Mile Square gets a bike-and-hip history, not a generic brace.

Questions patients ask

How is true sciatica different from other pain down the leg?

True sciatica in this clinic means nerve-root irritation, usually with pain in a dermatome and often with numbness, a reflex change, or weakness in a matching muscle. Other leg pain can come from the hip, the sacroiliac joint, a tendon, or a peripheral nerve and still feel dramatic. The name is earned by the exam, not by how far the ache travels.

Which exam findings make a lumbar nerve root more likely?

A pain map that matches one root, sensory change in that same strip, a reduced reflex, and weakness in the muscles that root supplies are the findings that carry weight. Nerve tension tests that reproduce the familiar pain, and then ease when the nerve is slackened, add support. One sore spot in the buttock does not replace that cluster.

Do I need an MRI before sciatica treatment can start?

Usually not, if the history is mechanical, the neurologic exam is stable, and there is no emergency feature. Movement testing can start while you watch how symptoms respond. An MRI moves up the list if weakness is worsening, if red-flag symptoms appear, or if a surgical opinion is already the decision in front of you.

What changes if my reflex or strength is not normal?

A clear strength or reflex change means the root is not just irritable. It is functionally involved. The plan becomes more cautious, retesting is closer together, and a physician visit is easier to justify, especially if the deficit is new or deepening. Exercise is still possible, but it is not casual, and spreading pain is not ignored.
