Timing, caffeine, and the nights pain wakes you
You might lie awake until 1 a.m., wake at 3 a.m. and stay up, or sleep enough hours and still feel unused. A large coffee after lunch, a bright screen in bed, and a schedule that shifts every weekend are ordinary explanations. They are worth fixing before a hormone kit.
Pain is its own alarm clock. A neck that cannot find a pillow, a low back that seizes when you roll, or a hip that throbs at night will fragment sleep no matter what your cortisol does. Neck pain is one of those mechanical reasons, and it deserves an exam.
A cortisol curve and a thyroid are different questions
Cortisol should rise after waking and fall by night. When the evening stays high, you can feel tired and unable to switch off. The adrenal fatigue page explains why FRI does not call that a burned-out gland. It is a rhythm question, tied to your story.
DUTCH hormone testing can map that curve when the history fits. A thyroid panel is a different test, used when you are also hot, cold, racing, or unusually slowed. One does not substitute for the other. Night sweats can be either story, or perimenopause, or the room.
This office is not a sleep laboratory
FRI does not run sleep studies, fit masks, or score apnea. If you snore, gasp, wake with headaches, or your partner watches you stop breathing, a physician should evaluate that. Say so early so a hormone plan is not built on top of an untreated breathing problem.
Dr. Sani Kohen, D.C. and Dr. Tara Kohen, D.C. can still look at caffeine, light, pain, and the labs named above. Chiropractic Sports Medicine can treat the neck or back that wakes you. Neuromuscular Rehabilitation can change the positions that fail at night. Those are not a sleep study.
What a visit changes, and what it leaves alone
Expect talk about bedtime, morning light, and the last caffeine. Expect an exam if pain is the alarm. A DUTCH collection is optional. Sleep drugs and psychiatric medication stay with the prescriber. Do not stop them to prove a point before a lab.
Functional medicine at FRI does not replace a primary physician, endocrinologist, gastroenterologist, or mental health clinician. If mood is the reason you cannot sleep, keep that clinician. Nutrition and supplementation is modest and specific, without a sedative protocol or a brand dose.
What to expect when you are seen for Sleep & Insomnia
You can call (949) 312-1754 during Monday–Thursday, 9–6, or book a complimentary 20-minute consultation. FRI is out of network. Superbills are provided, and HSA or FSA funds are accepted.
Bring a week of bedtimes, wake times, caffeine, and what hurts at night. If apnea is on the table, you will be pointed toward a physician. Testing, if any, follows the clue rather than a sleep package.
Where Newport patients usually go next on this site
People driving up from the peninsula and Corona del Mar often start with neck pain or shoulder pain after paddling and desk time, or with lower back pain after long sits in the car. Harbor athletes looking for a tissue exam rather than a generic rub sometimes book Active Release Technique or a gait and postural look when the foot and hip are part of the story.
Others are not here for a joint at all. Perimenopause and gut health questions are common alongside training, and the matching lab pages are DUTCH hormone testing and GI-MAP stool testing. If you want the wider map of the city, the Newport Beach chiropractor page ties the three pillars together. The doctors' backgrounds are on the about page.
What a Newport Beach week actually asks of the body
Newport Beach is not one neighborhood. Corona del Mar climbs away from the water, Balboa Peninsula is flat and windy, Newport Coast sits on hills above the toll road, and the streets around Eastbluff, Big Canyon, and Fashion Island mix short drives with long desk blocks. People paddle in the harbor, ride the Back Bay loop, play tennis, or swim before a day of meetings. That mix shows up in the history we take: which shoulder loads a paddle, which hip complains on the hills, which neck tightens after a commute that ends at a screen. None of those details is a diagnosis. They tell us which movements to test.
Food and schedule questions belong in the same visit when symptoms are not only mechanical. A late dinner after a harbor event, an early paddle, and a short night are ordinary Newport weeks. If you are also dealing with cycle changes, reflux, or a fatigue that does not match your training, the Functional Medicine side of the clinic can look at labs chosen from that story. The manual care and the lab conversation happen in the same practice, so you are not retelling the history in two buildings.
The drive up the 73 from Newport Beach
Most Newport Beach patients take the 73 north from MacArthur Boulevard or from Bison and stay on the toll road until the Mission Viejo exits that feed La Alameda. Door to door it is about 18 miles. Midday it can be close to 25 minutes. Late afternoon toward the coast, or the reverse commute when the toll road stacks, it stretches toward 40. The office is in a ground-floor suite at 26400 La Alameda, Suite 100, Mission Viejo, CA 92691, with parking at the building. Hours are Monday through Thursday, 9 to 6.
Plan the return before you book. A late-afternoon visit lands you back on the 73 toward Newport when the toll road is busiest. Morning and early afternoon slots are usually the calmer drive. Call (949) 312-1754 if you need to move a time. The first step for many people is a complimentary 20-minute consultation, which is a fit conversation, not a treatment squeezed into a sales pitch.
Questions patients ask
Can you diagnose sleep apnea?
No. FRI is not a sleep-medicine laboratory and does not diagnose apnea. Snoring, gasping, morning headaches, or witnessed pauses in breathing should be evaluated by a physician. The office can still address neck pain, caffeine, and a cortisol or thyroid question when those fit. Those steps do not replace a sleep study or a mask your physician prescribes.
Is poor sleep the same as adrenal fatigue?
No. Many people use the phrase adrenal fatigue for feeling wired and tired. FRI does not treat that phrase as a formal diagnosis and does not call the adrenal glands burned out. A DUTCH test can show a cortisol curve if your story supports it. Pain, caffeine, and schedule problems are often the louder findings, and they are handled directly.
What if my neck is why I wake up?
Then the neck is part of the sleep plan. Chiropractic Sports Medicine can examine the joints and the pillow problem. Neuromuscular Rehabilitation can retrain positions that collapse overnight. A hormone kit does not fix a neck that cannot rotate. If pain is new, severe, or follows an injury, say so, because that changes the exam.
Should I stop my sleep medication before a test?
Do not stop a sleep drug, a psychiatric medication, or any other prescription unless the prescribing clinician tells you to. FRI will explain collection instructions if a DUTCH test is ordered. Those instructions are not a taper. If mood or anxiety is the reason nights fail, keep your mental health clinician in the plan.
