Narrowing and the walk that runs out
People with symptomatic stenosis often describe a distance, a block or an aisle, after which the legs ache, tingle, or feel heavy. Sitting, or leaning on a cart, eases it. That shopping-cart relief is a clue to a flexion-preferring pattern. It is not, by itself, proof of the exact anatomy.
Central canal narrowing, lateral recess narrowing, and foraminal narrowing can produce different maps. Your report may name one. The exam still has to match the map to your walk. Lower back pain that is purely local and unrelated to distance is a different pattern and is not forced into this label.
Flexion relief and the circulation question
Neurogenic symptoms often change with posture: worse upright and extended, easier flexed. Vascular claudication is more about blood flow. It may arrive at a predictable walking distance and ease with standing still, without caring whether you lean forward. Pulses and that history are a physician question, not a chiropractic conclusion.
If your story could be vascular, you will be told to have that checked. Treating a circulation problem as a stiff spine is a mistake. If the posture change is obvious and vascular disease has been considered, flexion-biased care can proceed. Neurodynamics may be added when a root is clearly irritable.
Extension-sensitive movement choices
Long standing, prone press-ups, and overhead loads that throw you into extension often stir this pattern. They are not banned forever. They are poor opening exercises. The McKenzie Method is used to test directions, including flexion if that is what eases you, not to insist on extension because another disc problem liked it.
A directional test that makes the legs heavier or more numb is stopped. A direction that lets you walk farther is kept. That is the whole logic. Joint mobilization may help a segment that is stiff into the flexion you need for relief.
Conditioning while surgeons keep the decision
Walking tolerance, a stationary bike in a slightly flexed posture, and hip strength are common conditioning choices because they build capacity without living in extension. The dose is the distance you can cover without the legs declaring the walk over. Next-day heaviness means you overshot.
Whether you need decompression or another operation is the surgeon's decision. This office does not perform that surgery and will not talk you into or out of it with a slogan. New bowel or bladder changes, saddle numbness, rapidly worsening weakness, fever with spine pain, or significant trauma belong in emergency care, not a routine visit.
What to expect when you are seen for Spinal Stenosis
You will describe the walk that fails and the posture that restores your legs. Repeated movements test that story. You should leave knowing whether flexion eased the limbs and whether a vascular check is still missing.
Conditioning starts below your current limit and moves the limit gradually. Call (949) 312-1754 if you want to ask whether this fits before you book. No one will promise the canal will be wider because a bike ride felt better.
Santa Margarita Parkway to La Alameda
Santa Margarita Parkway into Mission Viejo, or Antonio Parkway to local streets, covers the trip. About 7 miles and 12 to 20 minutes when the parkway is moving. School traffic changes that. The suite is 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. Park at the building. Monday through Thursday, 9 to 6.
You are close enough to do a real course of care without giving up half a day. Use that. Spacing visits so you can practice the home plan is more valuable than stacking them. Call (949) 312-1754 or begin with the free 20-minute consultation.
Hands-on care and the lab side
Functional movement screening and gait analysis fit a town that hikes and then sits. ART is the soft-tissue option for overuse from sport, not a spa massage. Neurodynamics belongs when leg or arm pain behaves like a nerve.
For non-joint problems, sleep, hormonal imbalance, and gut health each have a page. The city starting point is the Rancho Santa Margarita chiropractor page.
Policies that do not change because you are close
Out of network. Superbill. HSA and FSA accepted. No referral required. We coordinate with physicians and we do not edit their prescriptions. Emergency spine and neurologic symptoms go to the ER.
Drs. Sani and Tara Kohen practice together at FRI. Read the about page and the office page if you want the people and the building before you drive Santa Margarita Parkway.
Dove Canyon, O'Neill trails, and a planned-town schedule
Rancho Santa Margarita spreads into Dove Canyon, Robinson Ranch, Tijeras Creek, and the central town. O'Neill Regional Park and the surrounding trails add climbing that a map of the shopping district does not show. Golf and youth sports fill weekends. Weekdays are often a drive out Antonio or Santa Margarita toward a desk somewhere else. Knees and hips meet the trails. Necks and low backs meet the car. We ask which village you live in because the grade and the drive are not the same in Dove Canyon as they are next to the lake in town.
Lower back pain after the drive out of the hills, and knee pain after O'Neill, are typical reasons to come to La Alameda. Scoliosis questions come from parents who want movement help without a promise that exercise will straighten a structural curve. We will not make that promise.
Questions patients ask
Why do my legs fade when I walk and ease when I sit?
Narrowing plus an upright, extended posture can crowd a nerve root or the cauda equina, so symptoms build with distance. Sitting or leaning forward opens that space a little for many people, and the legs ease. That is a clinical pattern. Imaging confirms anatomy. The pattern still has to be separated from a circulation problem that can also limit a walk.
How is this different from a blood-flow limit in the legs?
Neurogenic symptoms usually care about posture. Leaning on a cart helps, and standing tall hurts, even if you are not walking. Vascular claudication more often arrives at a set distance and eases when you simply stop, without needing to flex. Pulses and medical history matter. That distinction is a physician's call when the story is mixed. We will say so.
Why is bending backward often a sensitive direction?
Extension narrows the canal and the openings around the roots further, so legs can feel heavier, number, or weaker in that posture. It is a reason to avoid press-ups and long upright standing as your first exercises. It is not a lifetime ban. If a repeated-movement test shows a direction you tolerate, that direction is the one you practice.
Who decides whether stenosis should be treated with surgery?
A surgeon, using your exam, your imaging, and how limiting the walking has become. Functional Restoration Institute does not operate and does not pretend a exercise plan replaces that opinion when deficits are progressing. What we can offer is flexion-biased movement, mobilization, and conditioning aimed at walking tolerance, plus a clear note if emergency neurologic signs appear.
