Cuff load, stiffness, and neck referral
Pain on the outside of the shoulder during a lifting arc, with weakness in elevation or external rotation, points toward rotator cuff load. A global loss of motion, especially external rotation, with pain at the end of every direction, raises frozen shoulder.
Neck referral can sit on the shoulder blade or the upper arm and copy a cuff story. If looking toward one side or a cervical compression test reproduces the shoulder pain, the neck has to be examined before the cuff is blamed. Local shoulder tests still matter. They just do not go first when the neck already copied the pain.
Scapular motion and the thoracic spine
The shoulder blade has to rotate and tip so the ball of the joint has room when you lift the arm. If it wings or shrugs early, the cuff works harder for the same reach. Watching a slow arm raise from behind tells you more than a pain score.
The thoracic spine needs enough extension for that scapula to sit correctly. A stiff upper back is not the shoulder, and it still changes the shoulder. Joint mobilization may be aimed there, or at the glenohumeral joint, depending on which restriction matches your painful motion.
Matching today's shoulder exam to the story
A painful arc, resisted tests, and passive motion are compared with what you cannot do in real life: a bra clasp, a seatbelt, an overhead reach. The test that copies the daily limit is the one that leads the plan. Extra tests that do not change the decision are skipped.
Dr. Sani Kohen, D.C. will also ask about night pain and whether you sleep on that side. Night pain is common in cuff and capsular problems and is a poor guide by itself. Strength that is truly weak, not just painful, changes how hard loading can be in the first weeks.
What a shoulder plan actually tracks
The plan tracks range you care about, strength in the direction that failed, and whether the scapula still hikes. Soreness during a new exercise is judged the next day, not in the moment you dislike the effort. Spreading pain into the hand, or a sudden loss of strength, pauses the plan for a fresh exam.
Chiropractic Sports Medicine covers the joint work. Neuromuscular Rehabilitation covers the control. You will not be promised a date when overhead lifting returns. You will be shown the next load that your current motion can tolerate.
What to expect when you are seen for Shoulder Pain
The first visit is mostly watching you move and testing strength. Hands-on care, if it is used that day, targets the joint or region that reproduced your pain. You leave with one drill for the shoulder blade or the cuff, not a stack of bands.
Later visits remeasure the motion you lacked and progress the load only if the next-day response was acceptable. A complimentary 20-minute consultation is available if you want to ask which of the three patterns, cuff, stiffness, or neck, even fits before you start.
Westside shops, Eastside neighborhoods, and the 55
Costa Mesa is physically split. The Westside and the streets around the LAB and the action-sports shops mean standing, lifting, and a younger training culture. The Eastside and Mesa Verde are more residential. South Coast Plaza adds retail shifts on hard floors. The 55 freeway is the spine of the commute for people heading inland. Neck and low back complaints often belong to that freeway. Shoulder and knee complaints often belong to training or to a job on your feet. We ask which Costa Mesa, because the city is not one ergonomic story.
Action sports and gym culture also mean people arrive having already tried a lot of soft tissue. If you have had months of massage without a change in the movement that hurts, say what was actually done. ART and IASTM are specific tools. They are not a more expensive version of a general rub, and they are not always the right next step.
Reaching La Alameda from the 55
The usual drive is the 55 south to the 73, then toward Mission Viejo and La Alameda. From the Westside, some people prefer the 405 south to the 73. About 16 miles and 25 to 40 minutes. The address is 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. Parking on site. Open Monday through Thursday, 9 to 6.
Afternoon returns to Costa Mesa on the 55 are slow. A morning visit is kinder to a fresh neck exam and to your schedule. Call (949) 312-1754. If you work a retail shift at South Coast Plaza, bring that standing schedule to the history. It changes how we dose a plantar fascia or low back plan.
What Costa Mesa visits focus on
Shoulder pain, neck pain, and knee pain cover a lot of the training and work injuries. Whiplash after a freeway crash needs a careful exam and, often, coordination with other clinicians. Functional movement screening is useful for people who train hard and still cannot explain a recurring tweak.
Gut and hormone questions are welcome in the same practice. Food sensitivities and hormonal imbalance each have their own explanation so we are not pretending a adjustment will answer them. The Costa Mesa chiropractor page is the overview.
Questions patients ask
How do you tell rotator cuff pain from a stiff shoulder?
Cuff-pattern pain often shows up in a lifting arc and on resisted tests, while other directions of passive motion can still be relatively free. A stiff shoulder, including adhesive capsulitis, loses motion in a capsular pattern, with external rotation often hit hardest, and the limit is there even when you are not pushing against resistance. The two can overlap, so both passive motion and strength are tested.
Can my neck create pain that I feel entirely in the shoulder?
Yes. Cervical joints and nerve roots refer into the shoulder blade and upper arm, and the shoulder itself can be innocent. If neck movement or a cervical test copies the pain, the neck is examined and, when it is the driver, treated. Local cuff tests are still performed so a real shoulder problem is not missed just because the neck is also stiff.
Why does shoulder blade motion matter if the joint is what hurts?
The ball and socket depend on the scapula to get out of the way and to give the cuff a stable base. Early shrugging or winging means the cuff is doing extra work for every lift. Training that motion can lower the load on an irritable tendon. Ignoring it and only rubbing the sore spot leaves the same mechanics in place for the next reach.
What changes if my upper back will not extend?
A thoracic spine that stays flexed makes overhead motion harder and encourages the shoulder blade to tip the wrong way. Mobilizing that region can be part of shoulder care even though the pain is felt lower, in the shoulder. If thoracic motion is already fine, it is not treated just to complete a checklist. The shoulder joint and cuff then get the attention.
