A slow thyroid story is not a fast one
Hypothyroid symptoms tend to slow daily life. You may feel cold in a room others find comfortable, gain weight without a clear change in food, or fight constipation and dry skin. Hair can thin. Cycles can become heavier. Thinking may feel padded, and a nap does not fix it.
Hyperthyroid symptoms run the other direction. Heat bothers you, your heart races, your hands tremble, and weight can drop while your appetite rises. Sleep breaks. Bowels loosen. Anxiety can show up as a body feeling, which is why a heart and thyroid check belongs with your physician, not only a stress label.
Why TSH alone can miss your question
TSH is a pituitary signal telling the thyroid how hard to work. It is a useful screen, and it is not the whole conversation if you still feel cold, wired, or exhausted after being told the number is fine. The question you asked was about symptoms. A single signal from the pituitary may not answer it.
Free hormone levels describe what is available to tissues. Antibodies ask whether the immune system is involved. Reverse T3 asks whether hormone is being shunted aside during illness, calorie restriction, or heavy stress. FRI orders that wider look when your story does not match a one-line result.
What belongs on a full thyroid panel
A thyroid panel at FRI can include TSH, free T3, free T4, reverse T3, and thyroid antibodies. Each item answers a different question. TSH is the pituitary request. Free T4 is the hormone the gland releases. Free T3 is the more active form. Antibodies point toward an immune pattern.
Those names are not a score you pass or fail in this office, and FRI does not invent a private reference range. Results are read next to your heart rate, temperature, cycle, bowel pattern, and energy. Advanced bloodwork may add iron, vitamin D, or glucose markers when fatigue has more than one suspect.
Medication stays with the prescribing physician
If you already take thyroid hormone, dose changes belong to the clinician who prescribed it. Drs. Sani Kohen, D.C. and Tara Kohen, D.C. will not tell you to raise, lower, or stop that medication. Functional Medicine here can organize the panel, the history, and nutrition questions your prescriber may want to see.
Functional medicine at FRI does not replace a primary physician, endocrinologist, gastroenterologist, or mental health clinician. If nodules, a racing heart, or eye changes are part of the story, those need medical evaluation. You can also read how Hashimoto's thyroiditis differs when antibodies are the main finding.
What to expect when you are seen for Thyroid Dysfunction
A complimentary 20-minute consultation is the first step if you want to know whether FRI is the right office. Care is out of network. HSA and FSA cards are accepted, and a superbill is available when you submit a claim. The schedule is Monday–Thursday, 9–6.
Expect a history of energy, temperature, bowels, cycles, and prior thyroid results before any kit or blood draw is chosen. You review numbers in plain language. Nutrition may shift. No visit here rewrites a prescription.
Straightforward office policies
No referral required. Out of network. Superbill. HSA and FSA. We do not invent a copay. Labs, if ordered, cost extra and are explained first. Medication stays with your prescriber.
You will see Drs. Sani and Tara Kohen's practice, described on the about page. Photos of the gym and rooms are on the office page. If a problem is outside chiropractic and functional medicine, we will say so rather than stretch the visit to cover it.
Town Center days and Aliso Canyon edges
Aliso Viejo is compact, master planned, and hillier than it looks from the 73. The Town Center is errands, sitting, and short walks. The neighborhoods toward Aliso and Wood Canyons add dirt descents and stairs. A lot of residents commute north to Irvine business parks and then come home to a hill. Necks take the office. Knees take the canyon. We want both halves of that day, because treating only the part that hurts ignores the part that loaded it.
The canyon trails are a gift and a test. People start plantar fascia or knee complaints after they add mileage on dirt while their week is otherwise a chair. Gait analysis and a loading plan are more useful than a month of rest followed by the same trail.
Alicia, Pacific Park, and the 73
From the Town Center, Alicia Parkway toward Mission Viejo is direct. From the south and west edges, Pacific Park or the 73 north to a Mission Viejo exit also works. About 7 miles and 12 to 20 minutes when the parkways move. The suite is 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. On-site parking. Monday through Thursday, 9 to 6.
If your day already includes the 73 toll road to Irvine, stacking a clinic visit on the south end of that drive can be efficient. Do not book so tight that you walk in from a stressful drive and expect a meaningful neck exam. Call (949) 312-1754. The complimentary consultation is 20 minutes if you want to start smaller.
Care that fits a canyon-and-commute city
Neck pain, lower back pain, and headaches track with the Irvine commute. Hip and SI pain tracks with hills and carrying kids up them. Joint mobilization and corrective exercise are everyday tools. McKenzie care is the spine option when symptoms have a direction that centralizes them.
Hormone and gut visits are part of the same practice. Perimenopause and SIBO have their own pages so those questions are not buried under sports advice. The Aliso Viejo chiropractor page is the hub.
Questions patients ask
Should I stop my thyroid medication before testing?
Do not stop or skip thyroid medication for a lab unless the prescribing physician tells you to. Stopping on your own can make you feel worse and can distort the very panel you hoped would clarify things. FRI coordinates the timing questions with that prescriber. Bring the dose, the brand name you were given, and the last results you have.
What if my TSH is normal and I still feel slow?
That is a common reason people ask for a wider panel. Free T4, free T3, reverse T3, and antibodies can show a pattern TSH did not describe. A normal screen does not erase your symptoms, and an extra lab does not automatically mean you need a new drug. Your physician still decides on medication.
Can a fast thyroid feel like anxiety?
A racing heart, tremor, heat, and broken sleep can feel like anxiety even when the driver is thyroid hormone running high. That possibility deserves a medical check, not a guess. FRI does not treat anxiety disorders. If worry is the main problem, keep your mental health clinician involved while thyroid questions are sorted.
Do you adjust my dose from these labs?
No. Dose changes stay with the prescribing physician or endocrinologist. FRI can explain what TSH, free T3, free T4, reverse T3, and antibodies are asking, and can talk about food, iron, and sleep as context. Functional lab testing here does not replace the person who writes the prescription.
