Suction that lifts tissue layers
A cup is set on the skin and air is drawn out, by a pump or by a silicone squeeze, so the tissue rises into the cup. Massage pushes down. This pulls up. Skin, fascia, and the superficial muscle separate a little under that negative pressure. The goal is room between layers that have been stuck flat, especially before a joint needs to move through a larger range.
Cupping therapy at FRI is mechanical. Dr. Tara Kohen chooses the site from the movement exam, often next to a stiff shoulder blade or along a dense band beside the spine. The cup is not placed on a chart of organs. If the skin is broken, numb in a way that is new, or too fragile, the cup stays in the cabinet.
The circle of color and how it fades
A round mark is common. Color runs from pink to deep purple, depending on how much suction, how long, and how reactive your skin is. The color is blood in the superficial layers. It fades over several days, sometimes through green and yellow, like any other small bruise. It does not measure how many problems were solved.
We do not use cups to pull toxins out of the blood, the lymph, or the muscle. A darker circle is not a dirtier body region. If someone has told you the color is waste leaving, that is a different story from the one told in this office. You can ask for less suction when a visible mark would be a problem at work or at an event that week.
Decompression before you move
Cups often go on before exercise or before a slower soft tissue hold, when the tissue feels too dense to lengthen. After a few minutes the shoulder or the hip is retested. If the reach improved, the cup did its job for that hour. If nothing changed, leaving the cups on longer is not the next step. The plan moves to a joint technique or a drill.
Neck pain that sits in the upper back may get cups along the shoulder blade while the neck joints are handled separately. Myofascial release sometimes follows, because the lift makes a sustained hold easier to sink into. The pairing is about sequence. Cups are the opener, not the whole visit, in a Chiropractic Sports Medicine plan.
Still cups and cups that glide
Stationary cups sit in one place for a short time. Gliding cups move across oiled skin so the lift travels along a muscle. Gliding feels like a deep drag and covers more area. Stationary cups are more focal and more likely to leave a distinct circle. Dr. Sani Kohen picks from how irritable the tissue is. Very sensitive regions get shorter time and less suction.
You should not feel a sharp nerve pain, dizziness, or a symptom racing down an arm. Those are reasons to pop the seal immediately. Mild tightness under the cup is expected. Time is measured in minutes, not in an endurance contest. The skin is checked when the cup comes off, and the movement test is repeated before you get dressed.
What a Cupping Therapy visit is like
Oil or lotion may be used if the cups will glide. You will feel a pull, then a fullness, and you can ask for the suction to drop. A typical placement is brief. Visits often run 30 to 60 minutes depending on the plan, with cupping as one block inside testing and exercise. You will see the skin before you leave so the color is not a surprise.
Marks can last several days and then fade. Plan around photographs, competitions, or work dress codes if that matters to you. Light movement after is useful. The office is out of network, accepts HSA and FSA cards, and provides a superbill. A free 20-minute consultation is open if you want to ask how decompression would fit your training week.
Before you get on the 5
No referral is required. Bring shoes you hike or walk the trail in if the foot or knee is the issue. Prior MRIs are useful and not mandatory. If you have bowel or bladder changes, saddle numbness, or a rapidly weakening leg, do not drive past an emergency department to reach us.
The practice is out of network. San Clemente patients typically use HSA or FSA funds and a superbill. We will not guess your benefits. The doctors are Drs. Sani and Tara Kohen. Their biographies are on the about page, and the clinic layout is on the office page.
Pier Bowl, Talega grades, and Trestles mornings
San Clemente has two terrains that matter clinically. The Pier Bowl, the beach trail, and the streets near the water are relatively level and sandy. Talega, Forster Ranch, and the newer hills are the opposite: repeated grades, youth sports, and a drive just to reach I-5. Surfing at Trestles and along the pier loads shoulders and low backs. Hill neighborhoods load knees on the way down and necks on the drive. We ask which part of town you live in because the home program has to match the grade outside.
A lot of San Clemente patients are trying to stay active while working north of here. The I-5 commute itself is a sitting test. If your low back or sciatica eases when you walk the beach trail and worsens in the car, that detail changes the exam. Tell us the direction of the commute and how long you sit, not only where it hurts.
Coming north on I-5 to the clinic
The drive is I-5 north through San Juan Capistrano into Mission Viejo, about 16 miles. Without a slowdown it is a little over 20 minutes. A backup through the Capistrano stretch pushes it toward 35. Exit for the La Alameda side of Mission Viejo and come to 26400 La Alameda, Suite 100, CA 92691. Parking is on site. The doors are open Monday through Thursday, 9 to 6.
Because the drive is simple, people sometimes stack a visit onto a day that already includes a surf and a commute. Say if that is the plan. We may want the exam before the surf, or we may want you to skip the session that morning so the tests mean something. Call (949) 312-1754 if you are sorting the order. The 20-minute consultation can be the first appointment if you are not ready for a full evaluation.
Questions patients ask
Do the marks mean toxins left the muscle?
No. The color is superficial blood from the suction. It fades like a bruise. Darkness is not a measure of waste removed, and cupping is not used here as a cleanse.
How long does a circle last?
Often a few days, sometimes longer if the suction was stronger or your skin marks easily. It should fade, not spread or break the skin.
Can cups be placed on a painful joint itself?
Usually they sit on the soft tissue nearby, chosen from the movement test. A hot, swollen joint is not a place for strong suction.
Will I be sore after?
The area can feel worked, similar to a firm soft tissue session. Rising pain, numbness, or a mark that blisters should be reported.
