What shows up while you stand still
Standing is the first pass. Weight on one foot, a hip hitched, ribs shifted off the pelvis, a head that lives forward of the chest, knees that face different directions. They matter when they match a symptom. A posture that looks uneven and feels fine may be noted and left alone.
Gait and postural analysis does not require a force plate or a motion-capture suit. The doctor watches, may use a phone video so you can see the same thing in slow motion, and may palpate a landmark to confirm what the eye thought. If a fancier lab would be useful, that is a referral conversation. It is not pretended in the treatment room.
Watching the walk
You walk a short path, turn, and walk back, more than once. The look is at foot strike, whether the arch collapses, whether the knee stays over the foot, whether the hip drops as the other leg swings, and whether the trunk sways or stays rigid. Arm swing is part of it. A trunk that refuses to rotate often shows up here before it shows up in a story about stiffness.
Shoes change the picture, so the walk may happen in your usual shoes and briefly without them. Plantar fasciitis that is blamed only on the heel often looks different when the hip drop and the foot collapse are seen in the same stride. Knee pain on one side is compared with the other side's step, not with a textbook drawing.
Feet, hips, and trunk as one pass
Treating the foot and ignoring the hip, or treating the low back and never watching a step, is how plans get stuck. The pass is one chain. A stiff big toe can change the knee. A hip that will not extend can keep the low back working overtime at the end of the stride. The note says which link is loudest, and the first treatment goes there.
Dr. Sani Kohen will say the link out loud and show it on the video if video was used. You should be able to see your own hip drop or your own rigid trunk. Hip and sacroiliac joint pain during the stance phase is a reason to test that hip on the table next, not a reason to hand you generic posture cues. Cues without a finding do not stick.
How the notes rewrite the next exercise
The analysis is finished when it changes a decision. A collapsed arch may lead to a foot drill, a shoe question, or a look at the hip that is driving the collapse. A rigid trunk may lead to a walking drill with a little rotation, or to dynamic neuromuscular stabilization if the trunk cannot organize even lying down. The exercise has to resemble the fault that was seen.
Reassessment is another walk, not only a question about pain. If the hip still drops, the drill did not transfer. Visits often run 30 to 60 minutes depending on the plan, and the walking portion is a slice of that time, repeated after treatment. A free 20-minute consultation can cover whether a walking look is the right start. The office is out of network and can provide a superbill. HSA and FSA cards are accepted.
What a Gait & Postural Analysis visit is like
Wear shorts or pants you can move in, and bring the shoes you actually walk or run in, including orthotics if you use them. You will stand, walk, and possibly try a small single-leg task. Expect to watch a replay if video is used. The commentary is specific: this hip, this part of the step, this shoe.
You should leave knowing one fault and one thing the plan will do about it. Mild awkwardness on camera is normal. The visit is not a posture grade. FRI does not rank your stance against an ideal statue. The question is whether the way you load matches the pain you have.
Problems Laguna patients actually bring in
Downhill walking and stairs show up as knee pain and hip or SI pain. Ocean swimming and paddling show up at the shoulder. Neck pain after the drive through the canyon is common enough that we separate it from jaw and headache problems. Headaches that start at the neck are evaluated as a movement problem. A sudden worst headache is an emergency, not a chiropractic visit.
Functional questions travel the canyon too. People ask about sleep, perimenopause, and food reactions in the same season they are trying to keep hiking. Those visits start with history. A thyroid panel or food sensitivity test is ordered when it would change the plan. The Laguna Beach chiropractor page is the city hub.
What you should expect from a visit, not a package
Hands-on care might include joint work, soft tissue, and a few exercises chosen from the exam. Joint mobilization is often a better fit than a forceful adjustment when a joint is irritable, which is common after a flare from stairs. Corrective exercise is the part you repeat on your own so the hill outside your house stops being a surprise.
Payment is out of network, with superbills and HSA/FSA cards accepted. We will not quote your reimbursement. That is between you and the plan. Drs. Sani and Tara Kohen share the practice. Read their training on the about page and look at the rooms on the office page if you want to know the building before you drive the canyon.
Hills, stairs, and the village pace in Laguna Beach
Laguna Beach is a walking town built on grades. Stairs at the beaches, the climb through the village, Top of the World, and the canyon roads ask for knees and hips in a way the flat coastal cities do not. Artists, shop owners, and people who work from a house on a ridge often stack that climbing with long sitting. We want the specific hill, not a generic activity level. A knee that hurts on the way down Thalia is a different test from a knee that hurts after a desk day in Irvine.
The same ridges affect recovery. If we give you a walking program, the walk available outside your door may be steeper than the walk we had in mind. Say so. Tourist season also changes driving and standing: people who work the village in summer spend more hours on their feet. That belongs in the history next to any plantar fascia or low back complaint.
Questions patients ask
Do you use a 3D gait lab?
No. This is clinical observation, sometimes with simple video so you can see it. A laboratory study is a different service and would be a referral if it were truly needed.
What if my posture looks uneven but nothing hurts?
It is noted and not automatically treated. An observation earns a drill when it connects to a symptom, a limit, or a task you cannot do.
Should I bring orthotics and everyday shoes?
Yes. The walk in the shoes you live in is the relevant one. A few steps without them can show what the shoe was hiding.
Will you tell me to stand up straight?
Only if a specific cue changes the symptom or the step. A general order to stand tall, with no finding attached, is not the analysis.
