Cervicogenic headache versus other headache types
A cervicogenic headache tends to start in the neck or the base of the skull and spread forward, often on one side. Neck movement or a sustained posture reproduces it, and cervical motion is limited. That cluster is a pattern. It is not proof you cannot also have another headache disorder.
Migraine, tension-type headache, and less common vascular headaches follow different rules. They may include neck tightness without the neck being the source. If your history sounds like migraine, you will be told that directly and pointed toward the right conversation rather than folded into a neck-only plan.
Upper cervical, thoracic, and jaw screening
The upper cervical joints and the muscles at the skull base are examined because they can refer into the head. The upper thoracic spine is checked too, since stiffness there changes how the neck holds itself through the day. A headache that ignores all of those findings is unlikely to be a joint problem.
Jaw motion, clenching, and tenderness in the chewing muscles are screened because temporomandibular joint dysfunction can share a headache with the neck. Dental causes stay with a dentist. Neck pain care is added only when the neck actually reproduces the head pain.
Headaches this clinic does not claim to treat
Functional Restoration Institute does not claim to treat every headache type. Cluster patterns, headaches tied to medication overuse, and headaches driven by a medical illness need the physician who manages that illness. Chiropractic Sports Medicine can still examine the neck if a musculoskeletal contributor is a fair question.
Chiropractic adjustments are considered when an upper cervical or thoracic joint is stiff and provokes familiar head pain. They are not applied because a headache existed this month. If joint care does not change the headache, we say so and stop repeating it.
Sudden worst pain and neurologic deficit
A sudden worst headache of your life, a headache with a new neurologic deficit, fever with a stiff neck, or headache after significant head trauma belongs in emergency care. Do not book a routine visit for those. Call emergency services.
Ordinary recurring head pain that is clearly provoked by neck motion can be examined in the clinic. You will still be asked about vision change, waking from sleep with vomiting, and any headache that is changing its character. Those answers decide whether you stay here or leave for medical care.
What to expect when you are seen for Headaches
The visit is a sorting exercise. You describe the start site, the side, and what movement does, then the neck, upper back, and jaw are tested against that story. Treatment on day one, if any, is a low dose aimed at a joint or muscle that just reproduced the pain.
You should know before you leave whether the headache looked cervicogenic, mixed, or outside what this office should manage. Home care is specific to that answer. A complimentary consultation can be used to ask that classification question before a full course of care.
Old Town, the Legacy, and the North Tustin hills
Tustin has three different weeks. Old Town is walkable, with short blocks and a lot of sitting in cafes and small offices. Tustin Legacy, on the old base, is wide, new, and built around a car. North Tustin and the hills toward Foothill are grades, older homes, and a drive down to the 55. A low back from Legacy commuting is not a knee from a North Tustin hill. We ask which one you live in before we guess at a load.
The District and the shopping around the Legacy add standing errands on top of desk jobs. Neck pain and headaches often belong to the screen. Knee pain and hip pain show up when hill residents add steps without a plan. Plantar fasciitis shows up in people whose walking is almost all concrete between the car and a door.
South on the 55
The 55 south to the 73 is the usual path to La Alameda. About 16 miles. Twenty-five minutes is a good day. Forty is a 55 that is not cooperating. Address: 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. Parking at the building. Monday through Thursday, 9 to 6.
Do not book the visit as a gap between school pickup in Old Town and a meeting. The exam needs you present, not halfway back on the freeway in your head. Call (949) 312-1754. A complimentary 20-minute consultation is available if you want to meet the doctors before an evaluation.
Chiropractic, rehab, and labs from Tustin
McKenzie Method work is a strong fit when back or neck pain has a direction that makes it better or worse. Corrective exercise is the homework. Adjustments are used when a specific joint is restricted, after the screen that says a thrust is appropriate.
Functional medicine visits cover thyroid, sleep, and gut health when those are the actual complaint. The Tustin chiropractor page collects the local routes into the three pillars.
Fees, referrals, and the doctors
Out of network. Superbill. HSA and FSA. No referral required. We will not change a prescription from your physician. Emergency symptoms, including sudden neurologic loss, go to the ER rather than to La Alameda.
Drs. Sani and Tara Kohen practice at FRI. Their backgrounds are on the about page. The suite and gym are on the office page.
Questions patients ask
What makes a headache more likely to be cervicogenic?
Cervicogenic headache usually begins in the neck or skull base, often on one side, and neck movement or a held posture brings the same pain into the head. Cervical motion is limited, and examining the neck reproduces the familiar headache. Neck tightness alone does not qualify. Many other headaches include a sore neck without the neck being the driver.
Which headaches should go to emergency care instead of this clinic?
Go to emergency care for a sudden worst headache, a headache with new weakness, confusion, trouble speaking, or other neurologic deficit, fever with neck stiffness, or headache after major head trauma. Those are not routine musculoskeletal visits. A recurring headache that is clearly tied to neck movement is the pattern this office can examine.
Do you treat migraine, cluster, and tension headache as if they were the same?
No. They are different disorders, and this clinic does not claim ownership of all of them. A neck exam is still reasonable when you and the doctor suspect a cervical contributor. If the history fits migraine or another medical headache, you will be told, and any neck care is framed as an adjunct rather than the treatment of that disease.
Why examine the jaw during a visit that started as a headache?
The jaw and the upper neck share muscles and referral patterns. Clenching, limited opening, or tender chewing muscles can produce head pain that looks cervical until someone checks the jaw. Finding jaw involvement does not make it a dental diagnosis. Bite and tooth causes still belong with your dentist, who sees that part of the problem directly.
