First-step pain and plantar fascia load
The usual story is sharp pain at the inside of the heel on the first steps out of bed or after sitting, easing a little as you walk, then returning when you have been on your feet too long. That pattern fits plantar fascia load. It is still confirmed by tenderness at the medial calcaneal attachment and by how the big toe extension tightens the arch.
A sudden pop, bruising, or pain that is worse at night and not with first steps needs a different medical look. So does numbness and burning that follows a nerve rather than the fascia. Functional Restoration Institute treats the loading problem.
Calf and soleus capacity under the heel
Limited ankle dorsiflexion makes the foot compensate, and the fascia takes a share of that extra strain. The gastrocnemius limits the ankle with the knee straight. The soleus limits it with the knee bent. Both are tested, because stretching the wrong one feels busy and changes little.
Capacity matters as much as length. A calf that is flexible and weak still leaves the fascia doing work the muscle should share. Progressive loading of the calf and the foot is the center of care. Soft tissue work that is not followed by that loading does not hold.
Tape and instruments support a plan, they do not replace it
Kinesio taping can make the first steps more tolerable for a short time. It is support during a flare, not a fix, and it is not something you should need forever. If the pain returns the moment the tape comes off and strength never changed, the tape was doing all the work.
Graston and IASTM and Active Release Technique may be used on thickened, guarded tissue in the sole or calf when that tissue limits motion. They are irritable if overdosed. They are scheduled around the loading program, not stacked until the foot is too sore to train.
Footwear and the way the foot meets the ground
Gait and postural analysis looks at heel strike, how quickly the arch drops, and whether you push off through a painful inside heel. Shoes you have worn out, or a sudden change to a flat minimal shoe, are fair questions. They are not blamed before your walking is watched.
You may be asked to rotate to a shoe that matches the current irritability, then to reload the foot rather than hide in cushioning indefinitely. Neuromuscular Rehabilitation carries the loading plan. Chiropractic Sports Medicine covers the joint and soft tissue side when the ankle or foot joints are part of the limit.
What to expect when you are seen for Plantar Fasciitis
The exam checks the heel attachment, ankle motion with the knee straight and bent, and how you walk in your usual shoes. You will likely be given a small loading exercise and told how the next morning should feel. A worse morning means the dose was too much.
Tape or instrument work, if used, is explained as a short-term aid. Follow-up visits progress the calf and foot load and drop the aids when you can walk the first steps without them. There is no promised week when the heel will be silent.
Fees, referrals, and the doctors
Out of network. Superbill. HSA and FSA. No referral required. We will not change a prescription from your physician. Emergency symptoms, including sudden neurologic loss, go to the ER rather than to La Alameda.
Drs. Sani and Tara Kohen practice at FRI. Their backgrounds are on the about page. The suite and gym are on the office page.
Old Town, the Legacy, and the North Tustin hills
Tustin has three different weeks. Old Town is walkable, with short blocks and a lot of sitting in cafes and small offices. Tustin Legacy, on the old base, is wide, new, and built around a car. North Tustin and the hills toward Foothill are grades, older homes, and a drive down to the 55. A low back from Legacy commuting is not a knee from a North Tustin hill. We ask which one you live in before we guess at a load.
The District and the shopping around the Legacy add standing errands on top of desk jobs. Neck pain and headaches often belong to the screen. Knee pain and hip pain show up when hill residents add steps without a plan. Plantar fasciitis shows up in people whose walking is almost all concrete between the car and a door.
South on the 55
The 55 south to the 73 is the usual path to La Alameda. About 16 miles. Twenty-five minutes is a good day. Forty is a 55 that is not cooperating. Address: 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. Parking at the building. Monday through Thursday, 9 to 6.
Do not book the visit as a gap between school pickup in Old Town and a meeting. The exam needs you present, not halfway back on the freeway in your head. Call (949) 312-1754. A complimentary 20-minute consultation is available if you want to meet the doctors before an evaluation.
Chiropractic, rehab, and labs from Tustin
McKenzie Method work is a strong fit when back or neck pain has a direction that makes it better or worse. Corrective exercise is the homework. Adjustments are used when a specific joint is restricted, after the screen that says a thrust is appropriate.
Functional medicine visits cover thyroid, sleep, and gut health when those are the actual complaint. The Tustin chiropractor page collects the local routes into the three pillars.
Questions patients ask
Why do the first steps in the morning hurt so much?
After rest, the plantar fascia is loaded suddenly when you stand, before it has had a chance to ease into gait. Pain at the inside heel that is sharp on those first steps and eases as you move is the classic load pattern. It should still be confirmed by exam, because nerve pain and other heel problems can be worst at different times of day.
What do the calf and soleus have to do with arch pain?
If the ankle does not bend enough, the foot often collapses or the fascia takes extra strain to make up the motion. The gastrocnemius and the soleus limit that bend in different knee positions, so both are tested. A calf that is long but weak is a similar problem. Strengthening is what shares the load, not a single stretch held for a count.
Is kinesio tape a treatment for plantar fasciitis or just support?
It is support. Tape can make walking tolerable while you load the calf and foot, and it can be useful in a flare. It does not build capacity, and pain that exists only when the tape is on has not been solved. The plan should retire the tape as morning steps improve, rather than renewing it out of habit.
Can Graston or ART replace the strengthening for my heel?
No. Instrument work and Active Release Technique can change guarded or thickened tissue so the ankle and foot move more comfortably. They do not give the fascia or the calf the capacity to handle your day. Loading does that. If soft tissue sessions leave you too sore to do the exercise, the dose of hands-on work is reduced.
