Testing the joint before any thrust
The exam starts at the joint that will not move, and it compares that joint with the ones above and below. Direction matters. A block into extension is a different finding from a block into rotation. When someone arrives with lower back pain, the hips and the mid back are still checked, because the sore spot and the stuck spot are often not the same place.
Strength, reflexes, and sensation are recorded before any quick force. A joint that is swollen, freshly injured, or already very irritable may be the wrong candidate for a thrust that day. The same exam can point toward a slower joint mobilization instead. Screening is how the thrust stays specific.
What the quick thrust is doing
A chiropractic adjustment is a high-velocity, low-amplitude impulse through a segment chosen on the table. The speed is high and the distance is short, so the force stays local. The aim is to restore motion so the muscles crossing that joint can stop holding a guard. It is one segment, one direction, decided by the test.
People often feel a brief give, then less pressure in the area. Some feel very little and still show better motion when the joint is retested. Retesting is part of the visit. If the segment does not move better, the same thrust is not repeated out of routine. The finding chooses the next input.
The pop is gas in the joint fluid
The sound is cavitation. Joint surfaces separate, pressure inside the fluid drops, and dissolved gas comes out of solution. That gas is the pop. The bones are not striking each other, and a louder pop is not a better adjustment. A quiet thrust can restore motion. A loud one can miss if the wrong level was contacted.
Some joints make no sound and still improve on the retest. Patients hear this before the first thrust so the noise does not become the goal. If the sound is unsettling, hand position can change, or the table can assist, or the technique can slow down. Comfort and the motion test both count.
Screening, then soft tissue and exercise
Some histories remove a thrust from the plan. Recent fracture, signs of infection, a progressive loss of strength, or clear instability needs another route, and sometimes a physician. Neck thrusts get extra questions when the history includes vascular symptoms. Age by itself neither bans nor approves the technique. Dr. Sani Kohen or Dr. Tara Kohen makes that call from the screen.
Soft tissue often decides whether the new motion lasts. A restricted muscle can pull the segment back into its old pattern before the patient reaches the car. Hands-on work on that muscle is common in the same hour, followed by a short drill. Someone with neck pain may leave with one task for how they turn, matched to the segment that was treated.
What a Chiropractic Adjustments visit is like
Expect the exam before anyone sets up on the spine, a rib, or a limb joint. You will be told which segment is the target and what the thrust should feel like. The impulse itself is a fraction of a second. Visits in this style of care often run 30 to 60 minutes, depending on the plan, because testing and the follow-up drill share the time.
Afterward you may feel lighter, briefly tired, or mildly sore in the way a new movement can leave you sore. Walking is useful. A hard effort on that region can wait until the next day if the area feels stirred up. A free 20-minute consultation is open if you want to ask whether a thrust fits before a full visit. The office is out of network, takes HSA and FSA cards, and gives a superbill.
Chiropractic, rehab, and labs from Tustin
McKenzie Method work is a strong fit when back or neck pain has a direction that makes it better or worse. Corrective exercise is the homework. Adjustments are used when a specific joint is restricted, after the screen that says a thrust is appropriate.
Functional medicine visits cover thyroid, sleep, and gut health when those are the actual complaint. The Tustin chiropractor page collects the local routes into the three pillars.
Fees, referrals, and the doctors
Out of network. Superbill. HSA and FSA. No referral required. We will not change a prescription from your physician. Emergency symptoms, including sudden neurologic loss, go to the ER rather than to La Alameda.
Drs. Sani and Tara Kohen practice at FRI. Their backgrounds are on the about page. The suite and gym are on the office page.
Old Town, the Legacy, and the North Tustin hills
Tustin has three different weeks. Old Town is walkable, with short blocks and a lot of sitting in cafes and small offices. Tustin Legacy, on the old base, is wide, new, and built around a car. North Tustin and the hills toward Foothill are grades, older homes, and a drive down to the 55. A low back from Legacy commuting is not a knee from a North Tustin hill. We ask which one you live in before we guess at a load.
The District and the shopping around the Legacy add standing errands on top of desk jobs. Neck pain and headaches often belong to the screen. Knee pain and hip pain show up when hill residents add steps without a plan. Plantar fasciitis shows up in people whose walking is almost all concrete between the car and a door.
South on the 55
The 55 south to the 73 is the usual path to La Alameda. About 16 miles. Twenty-five minutes is a good day. Forty is a 55 that is not cooperating. Address: 26400 La Alameda, Suite 100, Mission Viejo, CA 92691. Parking at the building. Monday through Thursday, 9 to 6.
Do not book the visit as a gap between school pickup in Old Town and a meeting. The exam needs you present, not halfway back on the freeway in your head. Call (949) 312-1754. A complimentary 20-minute consultation is available if you want to meet the doctors before an evaluation.
How Chiropractic Adjustments is scheduled
An adjustment changes what the joint can do in the moment. It does not, by itself, teach a new habit. The hour usually continues into the tissues that cross the segment and into one drill you can repeat without equipment. Patients who only chase the cavitation often feel good on the table and stiff again after a long drive. Dr. Tara Kohen and Dr. Sani Kohen look for whether the new motion shows up when you stand, reach, or hinge. If it vanishes as soon as you load the area, the plan shifts toward exercise and soft tissue rather than another thrust at the same level.
People sometimes ask for a fixed series of adjustments. FRI does not sell a preset count. Response is rechecked, and care continues only while the exam still changes. Some joints need a slower technique for several visits before a thrust is even considered. Others move well once and then need the exercise more than they need more force. If you have been adjusted elsewhere on a schedule that never included a retest, bring that history. It helps the doctors see what has already been tried. A superbill is available if your plan reimburses out-of-network chiropractic care.
Questions patients ask
What does high-velocity, low-amplitude mean in practice?
The thrust is fast, and the joint moves only a short distance. Speed helps the segment give. The short distance keeps the force on the joint that was tested.
Why is there a pop if nothing was out of place?
Gas forms in the joint fluid when the surfaces separate. The pop is that gas. It is not bone hitting bone, and it is not a score.
Will every stiff joint receive a thrust?
No. Joints that are unstable, highly irritable, or unsafe on the screen are treated another way or referred.
Is this only for athletes?
No. Chiropractic Sports Medicine is the pillar, and the same exam is used for desk work, lifting, and sport.
